At CommunityCare's Member & Resource Center, you can:

Learn more about Medical & Prescription Plans – Come to one of our seminars or visit with a licensed representative one-on-one.
Receive help with enrollment – One of our licensed team members are available to assist you.
Receive customer service – We have Member Advocates here to assist you with benefits, premiums and claims questions.
No appointments necessary Monday through Thursday- you are welcome to drop in anytime!

Senior Health Plan is an HMO plan and a D-SNP plan with a Medicare & Medicaid contract. Enrollment in Senior Health Plan depends on contract renewal.

Please contact www.medicare.gov or 1-800-MEDICARE (1-800-633-4227) to get information on all of your options. Medicare customer service representatives are available 24 hours a day, 7 days a week.
