INTEGRIS Health Partners+ includes a Hospital at Home benefit.
Your INTEGRIS Health @ Home team consists of doctors, nurse practitioners, registered nurses, and may include other health care professionals such as physical & occupational therapists who will treat and monitor your health.
The length of stay for your Hospital at Home benefit is determined by your care team & the INTEGRIS Health Partners+ Medical Management team.
If you are able to participate you’ll have an individualized care plan that includes:
Eligible members will have a combination diagnosis of COPD, diabetes and/or congestive heart failure (CHF). INTEGRIS Health Partners+ and your INTEGRIS Health team will work with you to determine if you are eligible to participate in this program.
What is the difference between receiving hospital-level care at home or in a traditional hospital setting?
We Bring the Hospital to You.® The INTEGRIS Health @ Home care team’s goal is to provide high-quality hospital-level care in the comfort of your own home. These services include lab tests, mobile imaging like x-rays and ultrasounds, IV therapies, and durable medical equipment like shower chairs, grab bars, oxygen and hospital beds.
What is the cost of the program?
You have no copayments for the Hospital at Home program.
Will INTEGRIS Health @ Home work with my primary care physician?
Yes, we will work closely with your primary care physician to keep your care plan uninterrupted.
Who is my care team?
Your care team includes physicians, nurse practitioners, registered nurses, and may include other health care professionals such as physical and occupational therapists.
