Call our Customer Service team to request a claim form for your 2023 wallet expenses. See your Evidence of Coverage (EOC) for more information on your wallet benefit.
Examples include grab bars, toilet/tub safety rails, transfer benches, shower/tub benches & seats, and non-slip mats.
Don’t forget that you also have an Over-the-Counter (OTC) benefit. The OTC catalog includes some safety devices so you could save “wallet dollars” by using your OTC quarterly credit.
