Wallet Benefits

This is a reimbursement program that allows you to choose the benefits that you want! You can be reimbursed for your expenses for the benefits in your wallet up to the plan year maximum. Simply download the Wallet Benefit Claim form and submit it to CommunityCare with your proof of purchase. See your Evidence of Coverage (EOC) for more information on your wallet benefit.

Gold Wallet – plan year maximum of $500
Fitness/Gym memberships
Membership at a full-service health or fitness club; group classes for things like tai-chi, yoga, aerobics, etc. You can also be reimbursed for wearable fitness devices, such as FitBits.
Hearing Aids
No network – submit your receipt or proof of purchase and you will be reimbursed up to your available wallet maximum.
Personal Emergency Response System (PERS)
You can be reimbursed up to the plan maximum for a system which can provide access to calls for help in an emergency by pushing a button.
Home and Bathroom Safety Devices

Examples include grab bars, toilet/tub safety rails, transfer benches, shower/tub benches & seats, and non-slip mats.

Don’t forget that you also have an Over-the-Counter (OTC) benefit. The OTC catalog includes some safety devices so you could save “wallet dollars” by using your OTC quarterly credit.

Members on a Medicare Retiree group plan may have different benefits. Check your plan year benefit grid or call Customer Service with questions. Be sure to let us know what retiree group you are with.
